To finish the application process, bring the following documents to your appointment

1. Child’s original birth certificate with the raised seal or passport is acceptable.

2. ​Current immunization record with the dates of vaccinations given to your child.

3. Income verification for household:

   • Last year’s Income Tax Return, Form 1040 with supporting W2’s, 1099’s, etc.

   • Two (2) current consecutive paystubs for bi-weekly or bi-monthly payroll; four (4) current consecutive paystubs for weekly payroll.  Otherwise, a notarized letter on company letterhead  from employer stating your salary and hours.

   • A printout of your weekly benefits if you are receiving Unemployment.

   • The Allotment letter if you are receiving SSI benefits.

   • Letter from the Board of Social Services with monthly benefit amount if you are receiving public assistance

   • Verification of any child support payments you may be receiving.

4. Divorce decree (if applicable).

5. Social Security card(s) for all members in the family.

6. Three (3) proofs of residency that show your current address. 

   • A current electric/ gas

   • Telephone or cable bill.

   • Original Lease or Deed (if applicable).

7. Medical Insurance Card.​

8. Photo ID.
